Develop and maintain person-centered service plans based on each individual’s goals, strengths, and preferences
Connect individuals and families with community-based services, supports, and resources
Monitor services through monthly calls and quarterly visits and adjust plans as needs change
Assist individuals in understanding and managing DDD-funded budgets and services
Advocate alongside individuals and families to promote informed choice and independence
Collaborate with community agencies, providers, and statewide partners including DDD, DHS, fiscal intermediaries, and other support systems
Maintain timely documentation and compliance with DDD standards, policies, and deadlines
Build and maintain strong working relationships with consumers, families, providers, and community partners
Utilize technology and electronic systems to manage documentation, communication, and scheduling

\nQualifications

\nQualifications include:\n\n

Bachelor’s degree in Social Work, Psychology, Human Services, or a related field
Minimum of 1 year of professional experience supporting individuals with disabilities or related populations
Ability to speak proficiently in both Spanish and English to offer comprehensive support to consumers and their families.
Understanding of and commitment to the Independent Living Philosophy
Strong interpersonal, organizational, and communication skills
Ability to manage multiple priorities in a fast-paced environment
Experience collaborating with community agencies and service systems
Comfort using Microsoft Office and other technology platforms for daily work activities
Ability to work both independently and collaboratively as part of a team
Valid driver’s license and reliable transportation
Bilingual candidates are encouraged to apply

Salary: $53,500–$57,500 annually, commensurate with experience
